Orthopaedic ACL Reconstruction is a surgical procedure performed to repair a torn or damaged ACL in the knee.

Orthopaedic ACL Reconstruction

The ACL (Anterior Cruciate Ligament) is one of the major ligaments in the knee that helps provide stability by connecting the thigh bone (femur) to the shin bone (tibia). Injuries to the ACL are common, often occurring during sports activities or other traumatic events.

Here are some common types:

  • Autografts
  • Allografts
  • Synthetic Grafts
  • Double-Bundle Reconstruction
  • Anatomical Reconstruction
  • All-Inside ACL Reconstruction

Here is a general overview of the steps involved in ACL reconstruction:

Preoperative Assessment: Before the surgery, the patient undergoes a thorough preoperative assessment, including a physical examination, imaging studies (such as MRI), and discussions about the choice of graft and surgical technique.

Anesthesia: The procedure is usually performed under general anesthesia, although regional anesthesia (such as a nerve block) may also be used.

Arthroscopic Examination: The surgeon makes small incisions around the knee and inserts an arthroscope, a thin tube with a camera, to visualize the inside of the joint. This allows the surgeon to assess the extent of the ACL injury and identify any associated damage to other structures.

ACL Removal: The damaged or torn ACL is removed from the knee using arthroscopic instruments.

Graft Harvesting: If an autograft is chosen, the surgeon harvests the graft from a predetermined location.

Common autograft options include the patellar tendon, hamstring tendons, or quadriceps tendon. If an allograft or synthetic graft is chosen, the pre-prepared graft is used.

Graft Preparation: The harvested graft is prepared for insertion. In the case of a BPTB graft, bone plugs are often taken with the tendon. The graft's length and diameter are customized based on the patient's anatomy and the surgeon's preferences.

Tunnel Creation: Bone tunnels are drilled into the femur and tibia at the locations where the original ACL attaches. These tunnels serve as anchor points for the graft.

Graft Insertion: The graft is threaded through the bone tunnels and secured in place using screws, buttons, or other fixation devices. The graft is positioned to replicate the normal anatomy and function of the ACL.

Stability Assessment: The surgeon assesses the knee's stability by testing its range of motion and performing specific maneuvers to ensure that the reconstructed ACL provides the necessary support.

Closure and Dressing: The incisions are closed, and a sterile dressing is applied to the knee.

Recovery and Rehabilitation: The patient is taken to the recovery room and later moved to a postoperative care area. Physical therapy and a rehabilitation program are initiated to promote healing, restore strength, and improve range of motion.
