An appendicectomy is a surgical procedure that involves the removal of the appendix.

Appendicectomy

The appendix is a small, finger-shaped organ located at the lower right side of the abdomen. Appendicectomy is typically performed when a patient is diagnosed with acute appendicitis, which is an inflammation of the appendix that can be a medical emergency. The procedure is done to prevent the appendix from rupturing and potentially causing a serious infection in the abdominal cavity.

There are generally two main types of appendicectomy:

  • Open Appendicectomy
  • Laparoscopic Appendicectomy

Following are the procedure steps:

Open Appendicectomy:

Preparation: The patient is placed under general anesthesia, ensuring they are unconscious and pain-free during the procedure. Sterile drapes are used to cover the surgical area.

Incision: The surgeon makes a single incision, usually around 2 to 4 inches in length, in the lower right side of the abdomen. This incision provides access to the abdominal cavity.

Exploration: The surgeon carefully examines the abdominal cavity and locates the inflamed appendix.

Isolation: The appendix is isolated, and any adhesions or other structures that may be covering it are gently moved aside.

Appendix removal: The appendix is then ligated (tied off) at its base to prevent contamination, and it is then removed from the abdominal cavity.

Closure: The surgeon closes the incision with stitches or surgical staples. Sometimes, dissolvable sutures are used for the inner layers of tissue, while non-dissolvable sutures or staples are used for the outer skin layer.

Drains (if needed): In some cases, a drain may be placed to remove any excess fluid or infection from the surgical site.

Dressings: Sterile dressings are applied to the incision site to keep it clean and protected.

Laparoscopic Appendicectomy:

Preparation: The patient is placed under general anesthesia, and the surgical area is prepared as in an open procedure.

Incisions: The surgeon makes several small incisions (usually 3 to 4) in the abdominal wall, each about half an inch in size. One of these incisions is used to insert a laparoscope, and the others are used for specialized instruments.

Gas insufflation: Carbon dioxide gas is introduced into the abdominal cavity to create a space for the surgeon to work and to provide a clearer view with the laparoscope.

Exploration: The laparoscope is inserted through one of the incisions, and the surgeon examines the abdominal cavity to locate the appendix.

Appendix removal: Once the appendix is located, the surgeon uses specialized instruments to ligate and remove the appendix.

Closure: The small incisions are closed with stitches or surgical staples, and sterile dressings are applied to the incision sites.

Gas removal: The carbon dioxide gas is removed from the abdominal cavity.

Both open and laparoscopic appendicectomies have their advantages and are chosen based on the patient's specific condition, the surgeon's expertise, and other factors. Recovery time may be quicker for laparoscopic procedures, but not all cases are suitable for this minimally invasive approach. Always follow your surgeon's post-operative instructions for a successful recovery.
