Peritoneal dialysis (PD) is a form of dialysis used to remove waste products and excess fluids from the blood when the kidneys are no longer able to function adequately.

It utilizes the body’s peritoneum (a membrane that lines the abdominal cavity) as a filter to exchange waste, electrolytes, and fluids between the blood and a dialysis solution. This method is commonly used to treat patients with chronic kidney disease or end-stage renal disease (ESRD).

Here are some common types as below:

  • Continuous Ambulatory Peritoneal Dialysis
  • Automated Peritoneal Dialysis

Following are the procedure steps:

1. Catheter Insertion: A soft, flexible tube (peritoneal dialysis catheter) is surgically inserted into the abdomen, specifically into the peritoneal cavity. This catheter remains in place for dialysis fluid to be infused and drained.

The catheter is usually placed a few weeks before starting dialysis to allow time for healing.

2. Exchange Process:

Fill: Dialysis fluid (dialysate) is infused into the peritoneal cavity through the catheter.

The dialysate contains a sugar solution (usually dextrose or icodextrin) that helps to draw waste products and excess fluids out of the blood.

Dwell: The dialysate stays in the peritoneal cavity for a prescribed period (usually 4-6 hours during CAPD or several hours during sleep in APD). During this time, waste and excess fluid from the blood diffuse into the dialysate across the peritoneal membrane.

Drain: After the dwell period, the used dialysate (now containing waste and excess fluid) is drained from the abdomen into a drainage bag. The cycle then repeats with fresh dialysate.

3. Frequency of Exchanges:

CAPD: Typically, 3-5 exchanges per day with 4-6 hours of dwell time.

APD: Usually, 4-6 cycles are performed during the night, with a final exchange lasting through the day (optional for NIPD).
